Silver struck medal with obverse showing a scene, enclosed within corded border, of sailor being rescued by a female with legend: "LIFE SAVING MEDAL OF THE SECOND CLASS/UNITED STATES OF AMERICA." Reverse shows a laurel wreath within corded border bearing engraved inscription: "TO/JOHN H. RAPP/FOR/HEROIC SERVICES IN SAVING FROM/DROWNING IN THE EAST RIVER/NEW YORK/SEVERAL PERSONS/1877-1879" and surrounded by legend: "IN TESTIMONY OF HEROIC DEEDS IN SAVING LIFE FROM THE PERILS OF THE SEA."
