Bronze cast medal with obverse showing bust length-portrait of John Adams facing three-quarters to left; building at left of image in background; inscription at right: "JOHN ADAMS." Reverse bears the Great Seal of the United States with legend above: "IN GOD WE TRVST"; date below: "1776." Medal is inset into rectangular dark blue silk velvet presentation case with cream silk satin and dark blue silk velvet interior; gray outer cardbord box.
