Bronze struck uniface plaquette with rectangular panel depicting coat of arms in roundel labelled beneath: "SEAL OF PETER STUYVESANT", with distant harbor and cityscape in background at left; all enclosed by wreath of tulips; inscription in exergue: "JANUARY MCMXII"; beneath another inscription within scrolled band running across bottom of plaquette: "HOLLAND SOCIETY." Medal is inset into rectangular cream-colored cardboard box.
Inscription: edge, lower center: "WHITEHEAD-HOAG"
